ELIGIBILITY
A resident of the Dental Center of NW Ohio's service area living at or below 300% of the Federal Poverty Level OR a resident of the service area receiving health benefits from Medicaid or a Medicaid HMO is eligible for dental services.
MEDICAID, HMO'S & INSURANCE
The Dental Center accepts Medicaid and Medicaid-related HMO's, as well as most private insurance companies. For eligible families without Medicaid, HMO's or insurance, dental services for children under age 19 are billed at $25.00 per visit, regardless of the amount of dental treatment done (some exceptions may apply).
ADULTS
Adult fees are based on the current Medicaid reimbursement rates.
Proof of income (such as two recent paycheck stubs) will be required on the first visit to the Dental Center and annually thereafter.
