ELIGIBILITY
A resident of the Dental Center of NW Ohio's service area living at or below 400% of the Federal Poverty Level is eligible for dental services. Proof of income (such as two recent paycheck stubs) will be required on the first visit to the Dental Center, and annually thereafter.
SERVICES
Each year, tens of thousands of low-income children and adults receive routine dental care at the Dental Center of NW Ohio. Our services range from simple cleanings at one of our three fixed sites, to emergency extractions or dentures on our Smile Express. Our dedicated staff of licensed & skilled dentists, hygienists & dental assistants, as well as courteous patient relations representatives are always waiting with a smile.
PAYMENT The Dental Center accepts Medicaid and Medicaid-related HMO's, as well as most private insurance companies. For income eligible families without Medicaid or dental insurance, adult fees are discounted to Medicaid reimbursement rates. Dental services for children under age 19 are billed at $25.00 per visit, regardless of the amount of dental treatment completed (some exceptions may apply).
